Impact of HCV genotype and treatment regime

On the second day of APASL, Professor Peter Ferenci gave a lecture about the impact of HCV genotype and treatment regime on treatment of chronic hepatitis C. He said peginterferon alfa in combination with ribavirin was and would remain for the next years the standard treatment of chronic hepatitis C. The cure rate of peginterferon/ ribavirin treatment depends on host and viral factors. He emphasized that the genotype had the highest predictive value of cure rate, and also determined treatment duration and the dose of ribavirin needed. Measuring the viral response allows individualization of treatment duration. Treatment duration may be shortened in patients achieving rapid virologic response, on the other hand slow virologic responders may benefit from prolonged treatment.

In addition, he pointed out that response rates can be improved by treating conditions, such as bodyweight and alcohol abuse, which negatively influence the final outcome of therapy.
